I had to fill out a W-9 for a freelance project last year but did not receive any tax forms in return to file?

I had to fill out a W-9 for a freelance project last year but did not receive any tax forms in return to file my taxes with this year. The income was only $650 total, and it was a one-time design job. I have tried to get a hold of the person I worked with in this company and accounts payable with no success. They are a giant company responsible for many professional sports and concerts. Should I just claim it as "additional income?" or continue to try to get a hold of them? I want to file soon. Also, they did not take out any taxes for the $650 payment I received.
