Pain in my lower abdomen?

help please, my lower tum on the left side is hurting alot! its gone across to my back and its so painfull i feel sick, and i've taken painkillers but nothings working, ive been known to have constipation before but its never been this painfull and when i cough it hurts my lower tum!
